Neck Lift Overview

A Neck Lift, also known as Cericoplasty or Platysmaplasty, is a surgical procedure designed to restore elegance to your face. Though your neck isn’t technically part of your face, a neck with loose and sagging skin can age your look dramatically. Neck Lifts tighten your skin and the underlying muscles to lift your neck. It will also improve and sharpen the contour of your jawline. The procedure is beneficial for patients that have:

  • Vertical banding
  • Horizontal creases
  • Fatty jowls
  • The dreaded “turkey-wattle”
  • A crepe-like quality to the skin

A neck lift will give you a smoother, more toned appearance to the skin under your chin and the neck area that has lost its elasticity.

Immediately under the skin of your neck is the platysma muscle, which you can see in a mirror when you clench your teeth. When you are young, this paired muscle meets in the midline below your chin. As you age, the right and left sides of the platysma often separate, producing vertical ‘cords’ in the anterior neck. The platysma can be divided and tightened during Neck Lift cosmetic surgery. This helps permanently remove these ‘cords’ and improve your neck contour. Once tightened in the midline below your chin, lateral traction on the platysma creates a ‘sling”. It is this sling that elevates the soft tissues below your jawline. So if you have excess skin and fat beneath your chin as a result of genetics, advancing age, or weight loss, Neck Lift surgery can be the perfect solution.

Dr. Moelleken’s version of the interlacing neck lift is called the Neck-Lace, a procedure he created. This technique helps increase the firmness of your neck under your chin. As people age, many complain of a sagging neck, and the necklace procedure involves a small incision under your chin area. Like the liposuction technique, deposits are taken out of your jaw. These deposits cause sagging, and when removed, will improve the tightness of your neck skin.

Neck-Lace procedure is for patients with more fat and muscle looseness that can’t be handled with a corset platysmaplasty. This procedure involves extensive tightening of the muscle. There’s no volume reduction and no cutting of the platysma, which Dr. Moelleken says occurs from premature aging. Liposculpture is performed on the submental and jowl regions, as well. The incision is usually about one inch or less placed under your chin, and recovery is five to seven days. You should be able to return to your regular actives after two weeks, depending on your healing rate.

This potent procedure rejuvenates your neck with the more extensive tightening of the muscles and tissues above the glands. It will improve your look without removing any volume from the platysma muscle. Liposculpture of the submental and jowl region and upper neck is performed before the Neck-Lace Procedure. Dr. Moelleken believes this will be the new “platinum standard.” A procedure with superiority over corset platysmaplasty and a Neck Lift alone.

This procedure is for patients who want a complete rejuvenation of their face. It combines various methods, such as:

  • Dr. Moelleken’s signature LiveFill® graft insertion into your lips, nasolabial folds, marionette lines, and other customized areas
  • His signature Neck-Lace procedure for additional neck tightening
  • His signature USIC (Ultrashort Incision Cheek Lift) procedure for under your eyes and cheeks using fat preservation
  • Redistribution of lower eyelid fat, or LiveFill® grafts in patients with lower eyelid hollowness
  • A complete Bidirectional Face and Neck Lift procedure with 100% volume retention and no SMAS removal
  • Cheek augmentation with superior vector SMAS transfer;
  • An upper Lip Lift (gull wing)
  • Subcutaneous minimally invasive Brow Lift
  • Central minimally invasive nerve-sparing brow muscle reduction, upper blepharoplasty
  • Conservative laser resurfacing when required

Additional options to rejuvenate your neck include:

Minimally invasive devices such as radiofrequency (RF) can be used to heat your skin, produce new collagen, and tighten your skin. Such devices are the Accent, Venus Freeze, and Vela Smooth, in addition to Thermage, Polaris, and Re-Firme.

Some individuals have ‘sagging’ necks. This is primarily due to fat accumulation below their jawbone (mandible). In this case, when fat reduction is needed, liposuction or liposculpture of the area beneath your chin (submental region) or the jowls can be performed — resulting in a massive improvement to neck appearance. Liposuction is also frequently combined with surgical neck rejuvenation. In selected cases, some direct excision of fat (through a hidden incision below your chin) is performed. The incision is small (usually ¼ inch or less) and made under your chin with healing time approximately three days of wearing a dressing. The reduction of fat in the area below the jawline almost always makes the neck appear longer and makes your profile look more elegant or refined.

Wide Submental Liposculpture

Fat reduction and sculpting are performed through a small ¼ inch or less incision below your chin in the submental, jowl, and neck region. Recovery usually takes five days before you can resume your normal activities. It would be best if you made arrangements to have someone drive you home after your surgery.

Corset Platysmaplasty with Liposculpture

This technique is often combined with liposuction or liposculpture. It involves suturing together the platysmal muscles. This smooths the neck and is stronger than liposuction or liposculpture alone. It works best in patients who do not have a significant amount of skin laxity. The incision is usually placed one inch or less underneath your chin. Expect a full recovery in five to seven days. All patients heal differently, but most return to their regular activities in approximately two weeks.

Corset Platysmaplasty with Liposculpture Plus Face and Neck Lifting

This powerful treatment for your neck and jawline helps you achieve smoothing of these areas. Incisions are behind the tragus (bump in front of your ear), below your earlobe, behind the ear itself, and finally into your hairline. Recovery is ten days and two to three weeks before you can resume your regular routines. This is currently the gold standard for the rejuvenation of the face and neck.

Lower Facelift & Neck Lift Procedure

The neck lift is often combined with the lower facelift. Lower facelift being a laterally-based skin-tightening procedure.

While more extensive than a neck lift alone, this combination of a can be performed in the office as an outpatient procedure. The procedure is done under general anesthesia or IV sedation. The lower face and neck lift can even be done under local anesthesia with oral sedation in the appropriate patient. The process generally takes about one hour, depending on the scale of correction required.

During the procedure, the surgeon will make incisions in two possible spots. Either in the natural shadow line of the ear cartilage or behind the ear to minimize and hide post-op scarring. First, the skin and fat of the lower face is lifted, and the fascia vertically tightened, giving the face a permanent, natural-looking lift. Next, the vertical muscles of the neck (platysma) will be interwoven together at the center of the neck. Then a sling will be created to eliminate unsightly vertical banding. Any excess fatty tissue in is removed through liposuction, and any excess skin is tightened or removed. After the necessary corrections to the neck, permanent sutures or adhesive glue will maintain the desired shape.

Recovery from a lower facelift/neck lift will take between seven to ten days. We recommend avoiding brisk walking and other aerobic exercises for a month or so after surgery. Immediately after the procedure, most patients will notice bruising and swelling around the jaw, ears, and neck. This will subside over the next two weeks. Results will be visible immediately after the initial bruising and swelling diminish. Expect to see continuous improvement over the next 1-3 months as your body completely heals.
