Restylane® is the first and only cosmetic dermal filler made of a crystal-clear gel called NASHA, or Non-Animal Stabilized Hyaluronic Acid. Hyaluronic Acid is a sugar-based, water absorbing gel found commonly in plants, people, and animals. Hyaluronic acid is a natural substance that already exists in the human body used to provide volume and fullness to your facial skin. NASHA is biodegradable and completely biocompatible with human hyaluronic acid. There are a lot of anti-aging injectables on the market for the purpose of smoothing lines and wrinkles, adding volume, and filling in scars. Some are good for mild to moderate issues while others are used primarily to plump and add volume to your face. Fillers are made of different ingredients and work in different ways but the end result is still a younger looking face. The most popular products are BOTOX®, JUVEDERM™, Restylane®, and Perlane®.
BOTOX® is an injectable like the other products but it is not a facial filler. BOTOX® is derived from the botulinum toxin which is actually a neurotoxin. It works by inhibiting the muscle contractions in your brow and wherever it is injected, it freezes the muscles that cause wrinkles. The wrinkles BOTOX® treats are called dynamic wrinkles because they appear as a result of movement like facial expressions. Though it is a toxin, BOTOX® is safe to use and results typically last between 4 and 6 months.
JUVEDERM™ is a hyaluronic acid filler with the consistency of a gel to treat moderate to severe lines and wrinkles by adding volume under your dermis. There are three different density types available depending on the severity of your wrinkles. Common treatment areas that respond well to this thick filler are those deep lines from the corners of your nose to the edges of your mouth. The base for this filler is hyaluronic acid and results typically last up to nine months. Unlike the toxin injectables, this filler is used for static wrinkles, which are the ones that are always visible.
Restylane® also has a base of hyaluronic acid gel. It is used primarily to smooth out fine to moderate lines around your mouth and eyes and it is also a popular lip plumper. Since hyaluronic acid is not derived from an animal source, it is safe for use as an injectable. The results from this product last up to six months while the reduction of lines and wrinkles lasts up to nine months.
Perlane® contains no animal proteins so this limits any risk of an animal-based allergic reaction. One treatment lasts an average of six months and the FDA has approved the use of Restylane® and Perlane® for the creases around your mouth. Perlane® is also used extensively for improvements such as plumping your eyebrows, smoothing under your eye hollows, lip enhancement, softening the appearance of scars, and enhancing volume around your cheek, chin, and jaw. Side effects of Perlane® injections are typically mild, but can include temporary site redness, swelling, and tenderness.
During your consultation, Dr. Moelleken will evaluate your face and determine the severity of your lines and wrinkles as well as your skin elasticity. Which filler to use is determined on a case-by-case basis as you may need treatment for dynamic wrinkles, static wrinkles, or sometimes both.
In 1996, the biotechnology company Q-Med of Sweden created a genetically engineered version of hyaluronic acid (Restylane® and Perlane®). As mentioned before, hyaluronic acid is a polysaccharide, or natural sugar, that is found largely in your skin. Its primary function is to provide volume and pliability and the unique characteristic of hyaluronic acid is its ability to bind with water up to 1000 times its volume. This property makes it ideal as a filler agent because even while hyaluronic acid filler is being naturally degraded, it continues to attract more water to it and this allows it to hold its shape even longer. Q-Med makes three versions of hyaluronic acid; Restylane® fine line, Restylane®, and Perlane®. With all versions of hyaluronic acid, the higher concentrations are designed to fill larger and deeper areas.
Fillers include a variety of substances designed to be injected below your skin to decrease the appearance of wrinkles. Filling skin defects using different materials has been attempted for over 100 years, initially in 1893 when the patient’s own fat was used. In 1953 silicone was used, but this caused severe complications in some patients and since the 1950’s, collagen processed from cows was used and Zyderm was approved as a collagen filler in 1981. Some of the older fillers, like collagen, lasted only a few months and required skin allergy testing before it could be placed in the face.
Restylane® injections are administered by Dr. Moelleken’s in his office normally using local anesthesia. Depending upon the extent of the treatment, the procedure can take up to 30 minutes and you will be able to return home shortly after your procedure.

Results are fully evident within one week and Restylane® has a success rate for reducing or eliminating deep wrinkles that cannot be accomplished with other procedures such as chemical peels or microdermabrasion. Swelling may be pronounced but it lasts for only one to two days and normal activities can be resumed immediately although you will be advised to stay out of the sun. Restylane® treatments reduce wrinkles by approximately 80 percent and Restylane® is a temporary solution so follow-up treatments will be needed. The most common side effects of Restylane® include headache, nausea, flu-like symptoms, redness, and pain at the injection points.
