Laser Skin Resurfacing

Today, you have numerous choices when it comes to skin resurfacing procedures, from radio frequency to IPL to non-ablative procedures. In the past, ablative skin resurfacing with full-powered lasers was the only available solution for rejuvenating skin and erasing wrinkles. At that time, surgical CO2 lasers were used as a means to help skin recover from sun damage and discoloration. Ablative laser skin resurfacing worked by applying a very precise amount of laser energy to the skin, which heated up the water in the top layers to the point where water evaporated and the skin cells were destroyed. After that, the body’s natural collagen building repair system took over and built new layers of skin. Ablative skin resurfacing worked by removing sun-damaged skin and filled in new collagen where wrinkles and fine lines once existed. Thousands of women underwent this procedure but the complications and side effects were in some cases unbearable. Patients had to take up to seven days off from work, they could not go outside into the sun for several weeks, and the healing process involved an oozing, bubbling, and painful recovery. It was soon discovered that ablative laser skin resurfacing, although effective, was not ideally suited to most patient’s lifestyles and tolerance for pain.

A few years ago, scientists began exploring the idea of pulsing and pixelating lasers in such a way that they would damage the skin beneath the surface, which has the collagen-producing effects while leaving the skin at the top intact. This shift significantly reduces patient downtime and today these fractional CO 2 lasers have revolutionized the services cosmetic surgeons are able to provide. Fractional CO2 lasers employ a technique known as fractional photothermolysis, which uses precise laser beams to affect very small-interspersed groupings of skin cells. This method leaves the adjacent cells intact so that they can quickly fill in the very small damaged areas and significantly decrease the healing process. Fractional laser technology continues to evolve and today’s systems are much more effective and patient-friendly than ever before. One of the main benefits of fractional laser treatments is that they are generally very comfortable for the patient as most only experience a minor prickling sensation. When used with a topical numbing gel or air-cooling system, patients will only feel slight heat on their skin and there is usually no recovery time after a fractional procedure.

There are two types of fractional lasers:

  • The more gentle fractional lasers that use an erbium glass laser (like the original Fraxel)
  • The more powerful fractional carbon dioxide (CO2) lasers
  • The fractionated carbon dioxide lasers can treat more deeply, but they also can require more downtime than the more gentle, non-CO2 fractionated laser

Erbium Lasers

Recently, Erbium lasers have been developed which are safer and minimize patient downtime and recovery from skin rejuvenation. Erbium lasers were first used in the 1990s in dental procedures and then later for cosmetic treatments. Today, the devices are a very useful tool in the growing field of laser skin resurfacing. The Erbium laser uses the chemical element erbium to generate energy (hence the name) and fires infrared light onto your skin in short bursts. Erbium: YAG lasers emit extremely short pulses of infrared energy (2940nm). Tissue studies show that these lasers remove very precise thin layers of skin or tissue with each pass (pulse). In effect, an Erbium laser behaves similarly to a Dermabrasion device with significantly more control in the removal of skin layers. The Erbium laser results in a narrower area of heating of adjacent tissues than with other laser skin resurfacing methods and this results in a more focused removal of old skin cells layers.

Erbium laser procedures are most commonly performed on the face, but the laser can also be used on the neck, arms, and hands. The Erbium laser is the latest advance for the treatment of aging and sun-damaged skin. Erbium laser skin resurfacing is a procedure specifically designed to remove superficial and moderately deep lines and wrinkles on your face. The laser is ideal for treating age spots, acne scars, mole removal, tattoo removal, rhinophyma, and other such skin conditions.

The Erbium laser produces less heating or burning of surrounding tissues than other lasers used for resurfacing. This allows Dr. Moelleken to gently and comfortably vaporize the damaged skin cells layer by layer. In the Erbium Laser Resurfacing procedure a small area of the surface of the skin is heated with the cool Erbium laser which in turn vaporizes superficial damaged skin cells and resulting in the thickening of the skin’s collagen. Studies have shown that the Erbium laser stimulates the production of collagen underneath your epidermis, which fills in wrinkles while getting rid of dead skin at the same time. The Erbium laser provides a method of skin resurfacing that utilizes the cool light of the Erbium laser and this technique results in less post-procedure redness, swelling, and a greater turn around in healing time than with other laser skin resurfacing methods. All of this culminates in the Erbium laser being a more accurate and less painful method of restoring your skin to its more youthful appearance.

Pixel and Fraxel Lasers

Pixel and Fraxel are brand names for specific types of laser treatment systems. Both Pixel and Fraxel treatments utilize fractionated lasers to resurface your skin. Instead of one large laser beam, fractionated lasers create many small, separated microbeams, which hit the skin in such a way that the areas between the beams are unaffected. Because there are portions of the skin that are left untreated, the recovery time is faster and there is less chance of complications. The areas of skin that the laser does reach are stimulated, so new collagen production is promoted and the skin becomes plumper.

Fraxel Laser: Fraxel lasers are CO2 lasers that deliver more heat to skin tissue. This results in greater collagen stimulation for a more dramatic improvement.

Pixel Laser: Pixel lasers are Erbium lasers, which penetrate skin tissue less deeply than a Fraxel laser. Pixel laser therapy usually requires multiple treatments for optimal results. The Pixel module delivers results equal to ablative lasers but it eliminates possible side effects of ablative treatments. It also can be used on the face, neck, chest, arms, and hands, which is a much wider scope than the traditional technologies. Although there are a lot of laser instruments similar to the dot matrix laser in the market with different names, the Pixel Laser can be mainly identified into two kinds: one is Erbium Laser and the other is the Pixel Laser. The force of penetration of the Erbium Laser is the weakest because it can only penetrate through the epidermis. The Erbium laser can treat superficial skin but cannot heat dermal collagen. Both the Fraxel and Pixel lasers are used to treat aged or damaged skin and results vary depending on the intensity of treatment and the type of laser used.

The Pixel Erbium Skin Resurfacing Facts

The Pixel® Erbium-YAG laser is a new generation of skin resurfacing lasers with the proven effectiveness of ablative (deeper) skin resurfacing with the comfort and convenience of a non-invasive approach. The Pixel is an Erbium-YAG laser that is used in laser skin resurfacing and laser treatment and is the best you can look without the use of a CO2 laser or by means of surgical intervention.

The Pixel Erbium is a preprogrammed laser beam that passes through the patented Pixel micro optics lens and penetrates your skin. In fractional laser technology, the laser beam is split into a series of dots in a grid pattern, just like the pixels on a digital camera. The laser penetrates the skin deeply in the small spots, which produces a wound healing response and new collagen formation whilst leaving normal skin between the dots. This means that the skin remains intact without an open wound and is able to cover very quickly. It creates a micro-injury, or an ablative and thermal effect while leaving the surrounding tissue intact. The micro-injuries begin the process of healing through collagen growth and collagen remodeling. This in turn tightens the skin and improves the overall texture and tone. Improvement in your skin occurs in a two-fold process as the laser removes damaged tissue at the surface of your skin and also generates heat below within the dermis to stimulate collagen.

The Pixel Erbium Resurfacing Procedure

During your Pixel Erbium Resurfacing procedure, you will experience a mild sunburn sensation for about an hour and then you will feel virtually no discomfort. Your new, epidermal skin develops immediately and this process of skin repair involves various stages. Your skin will have a pinkish tone for five to seven days and this is a normal sign your skin is healing deeply. Your skin will have a bronze appearance that lasts anywhere from three to fourteen days, depending on the treatment level. Your skin will naturally and vigorously exfoliate as the reorganized epidermal skin replaces dead tissue. Flaking is similar to that of minor sunburn, without the associated pain and the use of a moisturizer will mask the appearance of flaking. Swelling is mild and generally resolves in two to three days. You will be required to use a sunscreen to protect your skin from sun exposure during the healing phase. Applying a moisturizing sunblock with an SPF of at least 30-plus is recommended.

The Pixel Erbium Resurfacing Procedure is performed with minimal downtime and discomfort. Unlike traditional treatments, with this latest technology you will not have to deal with bandages, thick ointments, or confine yourself to your house for weeks during the healing process. There are no messy topical anesthetic creams to apply prior to the treatment, no dyes applied to the skin, and best of all; the aftercare is simple and convenient. This gentler laser resurfacing treatment delivered by Pixel means you can go from your laser treatment to your usual lifestyle routine within hours.

The Pixel creates a smoother, tighter, improved skin texture after just one session because this technology has been shown to trigger a biological response, which leads to collagen remodeling. For optimal improvement, Dr. Moelleken usually recommends a series of treatments but this will be determined during your consultation depending on your skin needs. The Pixel laser resurfacing process is so gentle that the hard to treat areas once forbidden by traditional resurfacing procedures, can now be addressed. Pixel Erbium resurfacing is able to focus on areas without the major concerns of hyper or hypopigmentation as well as infection and significant downtime. The length of the Erbium laser procedure varies from 15 minutes to slightly more than an hour depending on your individual needs.
