Loose, sagging, wrinkled skin under your neck can be the result of aging, sun-damage, tobacco use, heredity, or weight loss. As the population ages, face and neck lift surgery (called rhytidectomy) has become the third most desired facial plastic procedure. With facial aging, the skin and tissues lose volume and loosen on your face and neck. Crow’s feet around your eyes and forehead lines also appear and deepen while your jawline becomes undefined and jowls begin to form. Lines in the front of your neck occur and laxity in skin, muscle, and fat under your chin leads to the appearance of multiple chins. Relaxation of neck muscles can result in vertical bands running from your chin to the base of your neck and neck fat tends to descend and pooch, even without aging. Unfortunately the fat under your chin is often hereditary and unaffected by dieting and weight loss. Younger individuals may derive benefits from isolated liposuction since their tissues are still elastic and can conform to the new thinner contour of the neck following fatty tissue removal. Older individuals however, who have undesirable excess fat and flaccid neck muscles, may require a neck lift or facelift following liposuction to re-drape and smooth out your redundant midline neck skin.

The Neck-Lace® Procedure
Over the years, most people become unhappy with the laxity of neck structures or the fullness in their neck area. Your face is the most prominent part of the image you display to the world. It is both your most visible and most valuable personal asset and the focal point for all interactions, especially first impressions. Like most cosmetic surgeons, Dr. Moelleken takes tremendous pride in his work and is committed to delivering exceptional results. There are a number of treatments available to enhance the appearance of your neck. They range from very minimal, in-office procedures with no down time, to more invasive surgery requiring anesthesia and time off for recovery.
The Neck-Lace® procedure helps increase the firmness of your neck and involves a small incision just under your chin area. A technique similar to liposuction is used to take out deposits under your jaw that cause the sagging of your skin and when removed, this helps improve the tightness of your skin. Liposculpture is performed on the submental and jowl region as well and the incision is usually about one inch or less. The Neck-Lace® Procedure involves extensive tightening of the muscle with no volume reduction and no cutting of the platysma, which Dr. Moelleken believes to be a part of premature aging. Recovery is typically one to two weeks depending on your specific treatment method and your body’s capacity to heal. If you do not have loose skin but you still have bands in your neck, you will still look tired and worn out. For women who do not yet need a facelift, Dr. Moelleken will perform a Neck-Lace using the same incision underneath your chin but instead of just tying the muscles together in the center, he actually ties them more widely almost like tying your shoe in an inner lacing stitch. This results in better correction of the muscles below your neck.

Neck-Lace Procedure with Face and Neck Lifting:
With the more extensive tightening of the platysma muscle and the tissues above the glands, this procedure is exceptionally powerful at rejuvenating your neck without removing any volume from the platysma muscle. Liposculpture of the submental and jowl region and the upper neck is performed before the Neck-Lace Procedure. Dr. Moelleken believes this will be new “platinum standard” with superiority of corset platysmaplasty and Face/Neck lifting alone.
The 360-Facelift® Neck-Lace Procedure with Bidirectional Face and Necklift, LiveFill® and Ancillary procedures:
This procedure is for patients who want complete rejuvenation of their face. The skin of your cheek and anterior (front) neck is comprised of three interconnected layers.
The three layers of the facial skin are:
- The superficial epidermal-dermal layer
- The underlying subcutaneous fat
- Beneath the underlying subcutaneous fat, a gliding membrane composed of fibro-elastic connective tissue and muscle. This gliding connective tissue membrane is called the superficial musculo-aponeurotic system, abbreviated SMAS.
The SMAS has multiple fibrous extensions that attach through the subcutaneous fat to the superficial epidermal-dermal layer of the skin, and these connections cause the three layers to move together as a unit. The SMAS attachment to the epidermal-dermal layer is particularly notable at the lines on each side of the upper lip and mouth or the nasolabial creases.
Additionally, there are other options available to rejuvenate your face and neck including:
Radiofrequency Techniques: Minimally invasive devices such as radiofrequency (RF) and (Thermage, Polaris, Re-Firme) can be used to heat you skin, produce new collagen, and tighten your skin.
Submental Liposculpture: When fat reduction is needed, liposuction or liposculpture of the area beneath your chin (submental region) or the jowls can be performed. This procedure can often be performed under local anesthesia. The incision is a small, under the chin, and the size of the cut is approximately ¼ inch or less. Healing time is usually three days and includes wearing a dressing.
Wide Submental Liposculpture: Fat reduction and sculpting is performed through a small ¼ inch or less incision below your chin in the submental area and in the jowl and neck region. Recovery usually takes five days before you can resume your normal activities.
Corset Platysmaplasty with Liposculpture: This technique often combined with liposuction or liposculpture involves suturing together the plastysmal muscles. This smoothes your neck and this technique is stronger than liposuction or liposculpture alone. However, it works best in patients who do not have a significant amount of skin laxity. The incision is usually one inch or less long under your chin. Recovery is typically five to seven days before you can resume your normal activities.
Corset Platysmaplasty with Liposculpture Plus Face and Neck Lifting: This is a more powerful treatment, achieving the smoothing of your neck and jawline. Incisions are behind the tragus (bump in front of your ear), below your earlobe, and behind your ear itself, and then into your hairline. Recovery is typically 7-10 days before you can return to your normal routine. This is currently the gold standard for rejuvenation of the face and neck.
