Wrinkled skin is associated, fairly or unfairly, with being old. Before deciding on a wrinkle treatment, it is important to understand the difference between facial wrinkles and what causes them. Once the underlying cause is identified, Dr. Moelleken can implement the appropriate wrinkle treatment, virtually turning back the hands of time for your face. Wrinkles (rhytids) may be categorized as either static (persistent, permanent) or dynamic (expression related) and wrinkle folds. Dynamic facial wrinkles occur due to movement or muscle activity in the affected area whereas static wrinkles gradually develop over time due to a combination of factors including age, genetics, sun damage, chronic movement patterns and lifestyle choices such as smoking.
Dynamic wrinkles are those that are present only with muscle contracture. The tightening of muscles that are attached to your overlying skin causes dynamic wrinkles. Each muscle contraction causes your skin to constrict, forming lines between the bulk of your muscles. When you are young, your skin springs back to its original position when you finish making the facial expression (at rest) but as you get older, the dynamic wrinkles remain on your skin. The most common locations for dynamic wrinkles are frown lines between your eyebrows, horizontal creases across your forehead or “worry lines”, lip wrinkles, chin puckers, “bunny lines” around the bridge of the nose, glabellar creases, neck bands, and periorbital lines extending from the outer corners of your eyes often referred to as “crow’s feet”. Weakening these muscles will cause a reduction in the severity of the facial wrinkles or can even prevent them from developing altogether.
Dynamic wrinkles respond very well to BOTOX® Cosmetic and Dysport treatments. Injections of BOTOX® Cosmetic and Dysport into the specifically targeted muscles will temporarily decrease the action of the muscles for three to four months eliminating wrinkles or causing a softer appearance to already formed dynamic wrinkles. Preventative injections of BOTOX® Cosmetic and Dysport can also be utilized before dynamic wrinkle formation occurs.
Often, dynamic wrinkles can become static lines through many years of muscle contraction and the thinning of skin that naturally occurs with aging the effects of sun damage and the glabella and upper lip lines are good examples of this. Multiple treatments of BOTOX® Cosmetic and Dysport before filler is best, as often these persistent lines soften with time eliminating the need for filler in this region. Another example of pre-treating a static wrinkle with BOTOX® Cosmetic and Dysport is the softening of the depressor anguli oris action at the outer corners of your mouth. This elevates the corners as well as making the melomental fold (frown line) appear less depressed. Dermal filler can then be injected to fill-in remaining marionette (frown) lines.
Static wrinkles are the wrinkles and folds that are present at rest or when you are not forming facial expressions. They develop as you get older because natural substances such as collagen, elastin, and the hyaluronic acid that provides your skin with structure and volume decrease with age. The loss of skin structure and volume means that the skin is less able to spring back to its initial position after facial expression and as a result, static wrinkles and folds form. Static wrinkles are caused from sun damage, smoking, genetics, poor nutrition, or from prolonged dynamic wrinkling. Static facial wrinkles are visible regardless of muscle contraction and this leads to the classic wrinkles that occur on your cheeks and around your eyes. Static wrinkles tend to develop in skin that has thinned as a result of natural aging. In order to best treat static wrinkles, a distinction has to be made between the different types. Fine skin lines that are diffuse and cover a wide area will respond well to superficial skin rejuvenation. Diffuse moderate static wrinkles may require more aggressive skin rejuvenation and simple moderate static wrinkles respond well to fillers and fat transfer. Techniques that increase the thickness of your skin or that remove the damaged skin are sometime effective wrinkle treatments for static wrinkles. Laser skin resurfacing and deeper chemical peels remove the damaged skin and stimulate new collagen formation to improve your skin’s appearance, elasticity and thickness.
Static wrinkles are those lines in the face that persist after muscle tension is relieved. Examples of static wrinkles include, glabellar lines (vertical lines between the brows), perioral lines (upper lip and smile lines), nasolabial folds, dimples at the outer corners of the mouth, and frown lines (marionette lines). Static wrinkles do not respond to treatment with BOTOX® Cosmetic and Dysport as well as with hyaluronic acid fillers so over the past five years, dermal fillers have become the popular treatment. The most commonly used products are Restylane®, Perlane®, and Juvéderm™. All of these fillers are based around hyalauronic acid, a naturally found component of collagen, which forms the basis of many types of tissue including your skin. These products are now synthesized using genetic engineering to produce an exceptionally pure product. Since the products are based on human derived hyalauronic acid, there is no longer a concern regarding products derived from animals. BOTOX® Cosmetic and Dysport Facts
BOTOX® Cosmetic and Dysport is the brand name for botulinum toxin A, a therapeutic muscle-relaxing purified protein compound derived from a botulinum bacterium called Clostridium Botulinum. In scientific terms, BOTOX® Cosmetic and Dysport is a sterile lyophilized form of botulinum toxin type A produced from a culture of the Hall strain of C botulinum and purified by a series of acid precipitations to a crystalline complex containing the toxin and other proteins. One of the greatest pharmaceutical breakthroughs of all time, BOTOX® Cosmetic and Dysport was originally approved by the FDA for optic muscle disorders but it was quickly recognized for its Cosmetic and Dysport value. Reducing the activity of the muscles that cause facial lines to form over time, physicians have been using BOTOX® Cosmetic and Dysport for years as a treatment to correct visible signs of stress and aging. The medication relaxes the muscles in your face that cause wrinkles and when injected in small doses in specific areas, BOTOX® Cosmetic and Dysport injections block nerve muscles responsible for the repetitive actions that over time cause your lines and wrinkles.
The softening of wrinkles using botulinum toxin injections are a long established treatment for anyone wanting to improve the fine lines and wrinkles caused by aging. BOTOX® COSMETIC AND DYSPORT was introduced to the world in the late 1980s by ophthalmologists who used it to treat spasms of the muscles that move the eye (strabismus), uncontrollable blinking (blepharospasm), or a lazy eye. When blocking the muscle contractions, physicians observed a softening of the patient’s local wrinkles. This led to clinical trials and in 2002 the FDA approved BOTOX® COSMETIC AND DYSPORT for Cosmetic and Dysport purposes. Since then, BOTOX® Cosmetic and Dysport has become by far the most widely performed Cosmetic and Dysport procedure. It is so popular that statistics show there has been an increase of 488% in the number of Botox injections done in United States. Specifically this number is based on 4.6 million Botox treatments in 2007 and under a million in 2000. As reported by the American Society for Aesthetic Plastic Surgery, in 2007 nearly 11.7 million Cosmetic and Dysport procedures were performed in United States. Muscles are one of those things that most of us take for granted but they are incredibly important for two key reasons:

Because muscles are so crucial, they are incredibly sophisticated. They are efficient at turning fuel into motion, they are long-lasting, they are self-healing, and they are able to grow stronger with practice. They do everything from allowing you to walk to keeping your blood flowing. which of course keeps you alive.
When most people think of “muscles,” they think about the muscles that you can see but there are basically three kinds of unique muscles in your body.
- Skeletal Muscle
- Smooth Muscle
- Cardiac Muscle
Skeletal muscle is the type of muscle that you can see and feel. When you work out and increase your muscle mass, skeletal muscle is what is being exercised. Skeletal muscles attach to the skeleton and come in pairs: one muscle to move the bone in one direction and another to move it back the other way. These muscles usually contract voluntarily, which means that you think about contracting them and your nervous system tells them to do so. They can do a short, single contraction (twitch) or a long, sustained contraction (tetanus).
Smooth muscle is found in your digestive system, blood vessels, bladder, your airways, and in a female, your uterus. Smooth muscle has the ability to stretch and maintain tension for long periods of time. It contracts involuntarily, meaning that you do not have to think about contracting it because your nervous system controls it automatically.
Cardiac muscle is found only in your heart and its function is endurance and consistency. It can stretch in a limited way like smooth muscle and contract with the force of a skeletal muscle. It is a twitch muscle only and contracts involuntarily.
BOTOX® Cosmetic and Dysport Uses

Other patients experience muscle spasms in their eyelids or other areas of the face so by injecting BOTOX® COSMETIC AND DYSPORT, it fills the space. Since there is no more hollow space, the muscles cannot spasm as before because matter surrounds them. For wrinkles in any area where there is not a muscle or areas where the wrinkle is present when the muscle is at rest, then BOTOX® Cosmetic and Dysport is not a good option. There has to be a muscle movement causing the wrinkle, line or fold for BOTOX® Cosmetic and Dysport to be effective. BOTOX® Cosmetic and Dysport (botulinum toxin type A) is successfully used to treat blepharospasm, strabismus, and cervical dystonia and these are all conditions that in some way involve spasms, involuntary muscle contractions. Within a few hours to a couple of days after the botulinum toxin is injected into the affected muscle(s), the spasms or contractions are reduced or eliminated altogether. During clinical trials, nearly 90 percent of patients noticed significant improvement in their appearance one month after treatment.

BOTOX® Cosmetic and Dysport Procedure
Cosmetic and Dysport enervation with BOTOX® Cosmetic and Dysport is a simple and relatively safe procedure administered by Dr. Moelleken in his Beverly Hills office. The treatment is minimally invasive because they are simple injections. Dr. Moelleken uses a needle filled with material that is injected into the various target areas of your face as determined during your consultation. Since muscles only have the ability to contract, BOTOX® Cosmetic and Dysport inhibits the release of acetylcholine at the joining site of the nerve to the muscle so that the muscle never gets the message to contract. This allows the skin above your muscle to stay relaxed and with repeated treatments, often the lines and wrinkles completely disappear leaving your skin smoother while giving you a well-rested appearance. The most common treatments Dr. Moelleken performs are BOTOX® Cosmetic and Dysport injections between your eyebrows, forehead, and along the sides of your eyes where crow’s feet form. BOTOX® Cosmetic and Dysport procedures cause only slight discomfort and potentially a small amount of swelling in the injection site which can last up to 24 hours.
The events that will occur the day of your procedure include:
- The area to be injected will be cleansed with alcohol swabs.
- No anesthetic is necessary; however, you may request a topical anesthetic cream for your comfort.
- Dr. Moelleken will ask you to make specific expressions to identify facial muscle contractions.
- Dr. Moelleken will mark near the injection sites with temporary ink.
- Dr. Moelleken will make several quick injections in the region to be treated.
- The treated area will be cleansed again.
- The entire treatment process takes approximately 10 to 15 minutes.
BOTOX® Cosmetic and Dysport procedures are one of the safest methods for helping men and women look younger. A full or partial face lift can eliminate fine lines and wrinkles on a permanent basis, but is considered surgery so more risk is involved. If this is your first time being treated with botulinum type A, Botox Cosmetic and Dysport, or Dysport, you can expect your results to develop within 5 to 10 days after treatment. Dr. Moelleken’s patients experience different results in regards to time and duration.
You will no longer be able to make the expressions that have created the lines resulting in the wrinkles on your face. The facial muscle activity varies on both sides of your face, causing an imbalance of your facial features and uneven lines and folds of your upper face. The muscles between your brows are very strong and return first after your treatments. In order to maintain a balance to these muscles, it is recommended that your BOTOX® COSMETIC AND DYSPORT be repeated every 3-4 months for the first year. Receiving injections on a regular three month interval causes your muscles to atrophy or weaken. The weakened muscles eventually shrink the problem area and fine lines and wrinkles fade away. If you do not repeat treatment, your appearance will return to its pre-treatment state.
